Summary:
The Hospice Continuous Care LPN is pivotal in identifying the physical, psychological, social and spiritual needs of the patient and family.
The LPN works under the supervision and direction of the RN Case Manager and initiates the appropriate interventions in support of the patient and family.
The LPN also follows a comprehensive and responsive plan of care.
Job Description:
Provide direct skilled bedside nursing care to patients.
Ensure and document care and interventions provided as well as appropriate use of medication, durable medical equipment and supplies.
Qualifications:

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
