Quality Metrics Specialist

Description1. Works within a team environment or individually based on the project needs
2. Conducts follow-up telephone calls to low and rising risk patients to eliminate identified care gaps (IE preventative call campaign), schedule wellness visits and follow-up visits to improve access to office services and increase patient satisfaction
3. Able to navigate different EMR systems to review, coordinate, and collaborate about patient care and needs
4. Supports continuum of patient care by identifying patients with gaps in care or in need of quality metric documentation prior to scheduled identified visits
5. Monitor and facilitate compliance with required quality measures for all ACO patients
6. Collect and report data as deemed necessary and by request of Director to analyze possible gaps in care, focus on quality improvement processes and to track status of patient care.
7. Able to review office charts to identify gaps in care and coordinate services and the care team to manage these issues
8. Provide monthly progress to payors as directed
9. Assist clinical teams with proper documentation/requirements for quality requirements
10. Serves as knowledge expert for quality metrics
11. Work closely with physician offices to communicate and review status of annual wellness visits, quality metrics, and high risk patients for effective care management
12. Special projects and other duties as assigned
RequirementsHS grad MA or LPN preferred. Bi-lingual Spanish preferred. Highly developed interpersonal skills , in order to interact effectively and motivate patients to change behavior when necessary. Must be proficient at multi-tasking and prioritization working in a high volume environment with little supervision. Ability to work independently as well as in a team environment. Must be organized and able to multi-task. Ability to work in stressful situations and be flexible to accommodate patient and family needs. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
